Service Request Forms

Please print the appropriate form, complete it and return it by fax or mail. A customer service representative will contact you when the form is received at our office. We look forward to being your service provider.

• Business Service Agreement (On Net) - This agreement is for Business Customers in our On Net Areas.
• Business Service Agreement (Other Areas) - This agreement is for Business outside of our On Net Area.
• Residential Service Agreement (On Net) - This agreement is for Residential Customers in our On Net Areas.
• Residential Service Agreement (Other Areas) - This agreement is for Residential Customers outside of our On Net Area.

Payment Forms

• Credit/Debit Card Authorization - A Credit Card payment can be authorized for a one time payment or for automatic monthly debits. Print, and complete the form (instructions), and fax it to our office. To ensure payment was received, please call after faxing the form to get your confirmation number.
• Automatic Bank Draft Request - An Automatic Bank draft can be authorized for payments to be automatically drafted from your bank account each month for the payment of your telephone and/or Internet account. The draft is done on the last business day of each month. Bank drafts can not be done as one time payments.
